Core Physicians Medical Group was born out of a much-requested need for mobile primary care for individuals who reside in Senior Living Communities throughout Orange County, CA and surrounding areas. Seniors will receive consistent weekly Medical Provider visits from our care team whose focus and goal are to optimize personal health and well-being. Our medical team and providers arrange for mobile delivery of medical care such as labs, x-rays, and primary care visits to the comfort of each resident’s apartment. This eliminates transportation and other barriers to attaining the highest quality of care. Core Physicians Medical Group is available 24/7. The in-community care model for Senior Living Communities includes weekly visits, lab work, annual wellness exams, referrals and coordination with specialists, medication refills and reviews, EKGs, x-rays, ultrasounds, and transitional and chronic care. Core Physicians Medical Group patients also have access to remote care monitoring and 24/7 access to our care concierge.

Supportive Care Medical Group (our sister medical group) of community-based Physicians and Nurse Practitioners bring Wellness and Supportive Care to your home or senior living community including palliative care visits to help with transitional care management, chronic disease management, pain and other symptom management, medications management, advance care planning, and a whole host of psychosocial assessment, social services referrals, and spiritual counseling that comprise a truly integrated whole-person approach that has proven to yield better outcomes, higher satisfaction and lower costs among our patient population. One of the most time-consuming process for being able to move into a Senior Living Community, is having a completed Physician’s Report, Medication/PRN Orders and other required medical documentation (such as a recent TB Negative Test). Core Physicians works with Senior Living Communities and their current Residents and Seniors who are in the process of moving into a senior living community with completing accurate reports within 72 hours or less.

Core Physicians Medical Group provides a personalized, telehealth experience and is available to all our patients.
